How your Anthem HDHP works

How you will be paying for medical care

  • You pay the full cost of most medical visits and services until you reach your deductible
  • Once you reach your deductible, insurance will begin paying 90% of your costs, while you pay 10% coinsurance
  • You will only pay up to your out-of-pocket maximum, which is the maximum you will ever pay (after which, insurance will cover all eligible medical costs at 100%)
  • Preventive care and virtual visits will be covered at 100% (see full list of preventive care on Anthem’s website)
  • In-network vs. out-of-network matters—out-of-network care is more expensive and may include balance billing

Example: If you are enrolled in employee-only coverage with an in-network deductible of $1,700.

  • If you have physical therapy appointments at $100 per visit, you pay the full $100 per visit until you meet your deductible of $1,700
  •  You’ll hit that deductible on your 17th physical therapy appointment and after that, you only pay 10% of $100 ($10) at your next appointment
  • Once you’ve paid $3,400 out-of-pocket for in-network care, all eligible in-network medical costs are covered at 100% so you don’t have to pay anymore until the next plan year

Free virtual care resources

These programs are available even before you’ve met your deductible, so feel free to use them as often as you need.

  • LiveHealth Online: Virtual doctor’s visits for any urgent care issues so you can get prescriptions without visiting a doctor’s office, all at no cost
  • One Medical (available as of 1/1/26): Access same-day or next-day virtual visits with your dedicated primary care provider, message with their care team, and help navigating in-person care (deductible applies to in-person non-preventive care)
  • Hinge Health: Virtual physical therapy program designed for ongoing pain or recovery support, all from home

Your HSA: what you should know

An HSA is a personal savings account to pay for eligible health care expenses tax-free now or in the future.

How your HSA works

  • UMG’s contribution to your HSA will be available January 1, 2026
  • You can also contribute pre-tax through payroll––you can change the amount at any time in your benefits account
  • You can use your HSA for eligible medical, dental, and vision expenses—full list here
  • Your HSA balance rolls over from year to year and stays with you even if you leave the company
  • You can get reimbursed for eligible expenses now or save your receipts and reimburse yourself later in life to allow your HSA funds to grow (there is no deadline to submit HSA receipts for reimbursement)

Using Your HSA With a Limited Purpose FSA (LPFSA)

If you also enrolled in an LPFSA:

  • The LPFSA will only cover dental and vision expenses
  • Dental and vision expenses will be deducted from your LPFSA first
  • Any remaining eligible dental and vision expenses will be pulled from your HSA
  • Medical expenses always pull from your HSA

Note: LPFSA funds typically do not roll over, unlike HSA funds, and expire on March 15 of the following year. Using LPFSA for predictable dental/vision costs first can help maximize your HSA savings.

Investing your HSA

  • Once your HSA balance reaches $2,000, you may begin investing those funds
  • Investing is optional and can help your HSA grow for long-term savings, allowing you to use those funds tax-free on eligible medical expenses now or in the future
  • You may register for a HealthEquity account and log in to begin investing
    • HealthEquity offers mutual fund options, including a range of Vanguard funds
  • You can still use your HSA funds for eligible expenses at any time, even if you invest part of it

    • What counts as preventive care?
      • You can see Anthem’s full list of preventive care at their website.
    • Are virtual care visits covered before I meet my deductible?
      • Yes, virtual care is covered at $0 cost to you. You may use programs like LiveHealth Online, One Medical virtual visits, and Hinge Health for free virtual care.
    • What happens if I go out-of-network?
      • Out-of-network care costs more with a higher deductible, coinsurance amount, and out-of-pocket maximum.
      • You’ll pay 100% until you meet the out-of-network deductible, then 30% coinsurance until the out-of-pocket maximum.
      • Some providers may bill more than the allowed amount (balance billing), which you will be responsible for paying.
    • Can I enroll in or change my HSA contribution?
      • Yes, you can enroll in and/or change your HSA contribution at any time in your benefits account.
    • Can I enroll in or change my LPFSA contribution?
      • You can only enroll and/or make changes as a new hire, during annual enrollment, or if you experience a qualifying life event.
    • Can I contribute to both HSA and LPFSA?
      • Yes, you can have both.
      • Use the LPFSA for dental and vision expenses first (eligible expenses list here). Remaining eligible expenses will pull from your HSA.
      • Medical expenses are paid from your HSA only and will not come out of your LPFSA.
    • When can I use the employer contribution to my HSA?
      • UMG’s contribution will be available starting January 1 of each year, or in the next month following your enrollment.
    • What counts as an eligible HSA expense?
      • Eligible expenses include medical, dental, and vision costs such as copays, deductibles, prescription medications, and certain over-the-counter items. See full list here.
    • How do I pay for care using my HSA?
      • Use your HSA debit card at the provider’s office or pharmacy.
      • You can also pay out of pocket and reimburse yourself later through your HealthEquity account (there is no deadline to submit receipts for reimbursement).
    • Can I invest my HSA funds?
      • Yes, once your HSA balance reaches $2,000. Investing is optional and allows your funds to grow for future health care costs. You can still access cash for eligible medical expenses anytime.
    • Can I keep my HSA if I change medical plans?
      • You can always keep your HSA funds for eligible expenses or investing.
      • If you switch to a plan without a high deductible, you cannot contribute to your HSA or receive employer contributions.
    • What happens to my HSA if I leave UMG?
      • Your HSA is yours to keep. The balance rolls over and remains available for future eligible expenses.
    • How can I check my HSA balance or manage investments?
    • What happens if I use my HSA for ineligible or non-medical expenses?
      • Under age 65:
        • You pay a 20% IRS penalty and income tax on the amount, per IRS regulations.
        • The IRS requires that you pay income tax on the amount, but no 20% penalty is needed.
      • Age 65 or older:
        • The IRS requires that you pay income tax on the amount, but no 20% penalty is needed
