When enrolled in UMG’s Anthem medical plan, you and your partner have coverage for one Maven Cycle of fertility treatment or preservation.

A Maven Cycle provides bundled coverage for a full round of treatment. Covered services may include:

• IVF and IUI
• Fertility testing and diagnostics
• Embryo fertilization and transfers
• Egg or sperm freezing, retrieval, and thawing
• One year of egg or sperm storage
• Medications related to covered fertility treatment

An infertility diagnosis is not required to receive fertility treatment or preservation coverage.

Your Anthem plan’s deductible, co-pay, or coinsurance applies to covered medical and pharmacy services.

Finding a fertility provider

Maven can help you find participating fertility clinics in your area. You must use a provider in Maven’s partner network for your treatment to be covered.

Live more than 50 miles from a Maven provider? Contact Maven Member Benefit Services at benefits@mavenclinic.com to request an exception to use an out-of-network provider. Maven will review your request and provide a decision within three business days.

Building a family through adoption, surrogacy, or donor tissue can come with significant expenses. Maven manages reimbursement for eligible expenses up to a $14,000 lifetime maximum.

This benefit is available to benefits-eligible employees and their spouse/domestic partner.
