Fertility medical coverage (Anthem members only)

Maven is the administrator for fertility and family building benefits, providing personalized, high-touch support for every stage of the family building journey. Whether you’re trying to conceive, adopting, pursuing surrogacy, or starting IVF, Maven makes it easier to access care, coverage, and guidance.

Coverage for fertility claims are available to employees and their spouse/domestic partner enrolled in an Anthem plan. All medical and pharmacy expenses will apply to your elected health insurance cost share (such deductible, coinsurance, copay).

What is covered

One Maven Cycle for fertility treatment and preservation

Bundled coverage for a full round of treatment, including IVF, IUI, egg/sperm preservation, diagnostics, medications, and more. A medical diagnosis of infertility is not required to receive fertility treatment or preservation coverage.

Covered services include, but are not limited to:

• Embryo fertilization and transfers
• Embryology diagnostic and screening tests 
• Egg/sperm freezing, retrieval, thawing, and 1 year of storage
• Medications related to covered family building journeys

Maven’s provider network

Maven partners with hundreds of high-quality clinics across the U.S. to offer Maven members discounted rates and excellent patient care. They can provide you with a personalized referral list of in-network providers in your area. You must use Maven’s partner network to qualify for coverage. 

If you live 50+ miles away from a Maven provider, you may request an exception to use an out-of-network provider by reaching out to Maven’s Member Benefit Services team at benefits@mavenclinic.com. Maven will review and provide their decision within 3 business days.

Family reimbursement

Maven will manage the reimbursement for adoption, surrogacy, and donor tissue expenses up to a $14,000 lifetime maximum. This is available to all benefits-eligible employees and their spouse/domestic partner.

Covered adoption expenses include but are not limited to:

• Court costs and legal fees
• Agency fees
• Eligible and related travel and lodging expenses

Covered surrogacy expenses include but are not limited to:

• Court costs and legal fees
• Agency fees
• Eligible and related surrogate medical expenses

Covered donor tissue expenses include but are not limited to:

• Frozen donor eggs and sperm
• Live donor egg and sperm cycles
• Agency and legal fees

Virtual care and support

Alongside fertility coverage, Maven also provides unlimited, on-demand support from 35+ specialties in women’s and family health—all available by video chat or messaging.

Your dedicated care advocate

Partner with Maven for access to your dedicated care advocate who is available 24/7 to answer your questions, recommend Maven providers, and help you understand your bills.

There are many paths to parenthood, so Maven offers the following support based on where you are in your journey:

• Family planning support including nutritionists, career coaches, mental health providers, midwives, OB-GYNs, doulas, loss support, and more
• Egg freezing & fertility support including access to a network of clinics, educational resources, on-demand support from OB-GYNs, nutritionists, reproductive endocrinologists, and community support from others experiencing a similar journey
• Adoption & surrogacy support including help navigating agency, financial, and legal options, educational resources, on-demand adoption coaches and mental health providers, and access to a network of inclusive agencies
• Maternity support including pediatricians, lactation counseling, infant sleep coach, back-to-work support, and weekly action plans for each stage of pregnancy and clinical programs for high-risk pregnancies

2. Activate the Maven Wallet in the app—this is your home base for tracking your medical benefits and claim expenses
