Limited Purpose FSA
The Limited Purpose Flexible Spending Account (LPFSA) is available to employees enrolled in the Anthem High-Deductible Health Plan (HDHP).
Overview
You can contribute up to $3,300 pre-tax per calendar year to your limited purpose FSA for eligible dental and vision expenses. Since the FSA is a use-it-or-lose-it account, be sure to plan carefully so you do not end up forfeiting unused funds at the end of the plan year (March 15 of the following year).
You can enroll as a new hire, during Open Enrollment, or if you experience a qualifying life event during the year. Visit the benefits portal to enroll and select your annual election amount.
How It Works
You elect the amount you want to contribute to the limited purpose FSA per year, and your paycheck is deducted based on that amount divided into the number of pay periods remaining in the year.
While deductions are made in each paycheck, the entire amount is available to you as of January 1 (if elected during open enrollment) or as of your event date (if you have a qualifying life event).
You have two options to access your FSA:
- Pay doctors, pharmacies, and other providers directly, using your FSA debit card (this is mailed to you a few weeks after your enrollment)
- You may request a separate debit card for your dependents using these instructions
- Pay out of pocket and submit itemized receipts in your WageWorks account or in the WageWorks app (iOS / Android)
Eligible Expenses
You can use your LPFSA to pay for eligible dental and vision expenses that aren’t fully covered by insurance for you and your tax dependents, such as dental co-pays, dental services, orthodontia, glasses, contacts and contact solution, and more. You can see a comprehensive list at this link.
Important: Claims are subject to review by WageWorks to ensure they are qualified expenses and may require supporting documentation such as a letter of medical necessity from your doctor. If you have questions about expenses, contact WageWorks at (877) 924-3967.
Plan year
You must use your FSA balance for eligible expenses incurred within the specified timeframe. If you have remaining funds from the previous year, the plan provides a grace period of 2.5 months to use up those leftover funds (March 15 of the following year).
- For 2025: January 1, 2025 – March 15, 2026
- For 2026: January 1, 2026 – March 15, 2027
Deadline to Submit Claims
You must submit your itemized receipts for any prior year funds by April 30 of the following year. For example, any remaining 2026 FSA funds need to be submitted no later than April 30, 2027.
- For 2025: April 30, 2026
- For 2026: April 30, 2027
Register for an account
Once you have enrolled, you will need to register for an account on WageWorks (instruction PDF). It will take up to 1-2 weeks from the time you enroll until your information is updated in the WageWorks system.
If the system does not recognize you when you first try to register, please double check the information in Workday is correct. The ID code will be the last 4 digits of your social security number.
Debit card use
You will receive a debit card to use for eligible expenses. The debit card eliminates the need to pay for qualified expenses out of pocket by directly accessing the funds in your healthcare FSA.
You can use your debit card to pay for office visit co-pays, prescriptions at the pharmacy, and other eligible expenses at the point of service. You can also request additional cards for your eligible dependents using these instructions.
Important: Hold on to your receipts for these debit card expenses in case WageWorks needs additional documentation to prove your purchase. You can provide the requested information to WageWorks online
Failure to provide requested documentation on your debit card purchases may result in the suspension of your card.
Use it or lose it
Per IRS regulations, any balances remaining in the account after the plan year deadline must be forfeited. This means that if you do not use all your FSA funds by the end of the plan year grace period (March 15 of the following calendar year; for example, March 15, 2027, for 2026 elections), it will not be carried over and you will forfeit these funds.
