Open Enrollment | October 20 – November 24, 2025
Open enrollment is the yearly opportunity to:
- Make changes to your insurance plans
- Add eligible dependents to your coverage like dental and vision
- Contribute to a Flexible Spending Account (FSA) for 2026, which requires active re-enrollment each year
- If you enroll in the Anthem HDHP, you can only enroll in an HSA and/or LPFSA (more details below)
If you don’t want to make any changes, no action is needed. All changes will be
effective January 1, 2026.

instructions) - Or call the UMG Benefits Service Center at 888-526-2794 to make changes over the phone
Enroll in a Flexible Spending Account (FSA) for 2026
To elect a Healthcare and/or Dependent Care FSA for 2026, you must actively enroll during annual enrollment. Any 2025 FSA enrollments do not automatically renew in 2026.
Ensure you and your dependents are covered
Your current benefits automatically continue, so there is no action needed if you don’t want to make any changes and don’t want to enroll in an FSA. However, this is a good time to ensure your dependents are enrolled in any dental or vision insurance.

Anthem High-Deductible Health Plan (HDHP)
Getting Started
This plan has the lowest premiums, but you’ll pay full cost for most services until you meet your deductible (except preventive care is covered at 100%). After that, you pay coinsurance costs for each visit.
The Anthem HDHP may be a good fit if you:
- Want lower paycheck premiums
- Typically use fewer healthcare services
- Want the flexibility of visiting in- and out-of-network providers
- Offset your out-of-pocket costs with pre-tax money through a Health Savings Account (HSA)
- You can contribute up to $3,550 for employee only coverage / $7,050 for family coverage
- If you’re 55+, you can contribute up to an additional $1,000
- UMG will make an additional HSA contribution of $850 for employee only coverage or $1,700 for family coverage
- You can contribute up to $3,550 for employee only coverage / $7,050 for family coverage
Quick snapshot of costs:
- Lower per paycheck premiums
- In-network deductible: $1,700 individual / $3,400 family
- In-network visits: The plan pays 90% after you meet the deductible, with your responsibility at 10%
- Out-of-network deductible: $3,400 individual / $6,800 family
- Out-of-network visits: The plan pays 70% after you meet the $3,400 individual / $6,800 family deductible.
- In-network out-of-pocket maximum: $3,400 individual / $6,800 family
- Out-of-network out-of-pocket maximum: $6,800 individual / $13,400 family deductible.

Health Savings Account (HSA) with HDHP
A Health Savings Account (HSA) is offered to employees enrolled in the Anthem HDHP. An HSA is a tax-advantaged account that is all yours to use toward current or future health care expenses.
Highlights:
- Offset your out-of-pocket costs with pre-tax money through a Health Savings Account (HSA)
- You can contribute up to $3,550 for employee only coverage / $7,050 for family coverage
- If you’re 55+, you can contribute up to an additional $1,000
- UMG will make an additional HSA contribution of $850 for employee only coverage or $1,700 for family coverage
- You can contribute up to $3,550 for employee only coverage / $7,050 for family coverage
- No use-it-or-lose-it: Your unused HSA funds roll over year to year and go where you go
- No federal tax on HSA contributions, interest and investment earnings, withdrawals on qualified health expenses (state taxes will apply to participants in CA and NJ)
- You can invest your HSA money, so it grows over time

Limited Purpose Flexible Spending Account (LPFSA) with HDHP
- Available if you enroll in the Anthem High-Deductible Health Plan (HDHP) and contribute to an HSA
- Contribute up to $3,400 pre-tax to pay for dental and vision expenses only, such as glasses, contacts, dental cleanings, orthodontia, and more (see full list here)
- Account is use-it-or-lose-it, with a grace period until March 15 of the following year to spend unused funds

Fertility medical coverage managed by Maven (Anthem members only)
Fertility claims will be managed by Maven, offering 24/7 concierge support, expanded provider access, and a cycle-based coverage model (instead of a dollar cap) for more flexible, comprehensive care.
All medical and pharmacy expenses will apply to your elected health insurance cost share (such deductible, coinsurance, copay). This coverage is available to employees and their spouse/domestic partner enrolled in an Anthem plan.
What is included:
- Embryo fertilization and transfers
- Embryology diagnostic and screening tests
- Egg/sperm freezing, retrieval, thawing, and 1 year of storage
- Medications related to covered family building journeys

Family building reimbursement through Maven
Maven will manage the reimbursement for adoption, surrogacy, and donor tissue expenses up to a $14,000 lifetime maximum. This is available to all benefits-eligible employees and their spouse/domestic partner.
Covered expenses include but are not limited to:
- Court costs and legal fees for adoption and surrogacy
- Adoption and surrogacy agency fees
- Frozen/live donor eggs and sperm
- Eligible and related travel and lodging expenses

Group Personal Excess Liability Insurance
Group Personal Excess Liability Insurance helps protect your personal assets and future earnings. This coverage adds an extra layer of protection on top of your existing auto, homeowners, or renters insurance—providing higher limits if you’re ever held liable for damages or injuries.
The policy helps cover major claims or lawsuits that exceed your primary insurance limits, offering peace of mind knowing you’re protected against large, unexpected expenses. This insurance is employee-paid, covering you and eligible household members.
